29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  a) On or about August 23, 2018 at 21450 CR 12 South, Foley, Al; snaphook connected directly to lanyard: The employer did not ensure employees using personal fall arrest were properly anchored in that their rope was wrapped around a support beam/trusses and the snap hook was connected back to the rope while taking measurements and installing sheathing which exposed them to falls of over 30 feet.

29 CFR 1926.502(d)(9): Lanyards and/or vertical lifelines did not have a minimum breaking strength of 5,000 pounds (22.2 kN):  a) On or about August 23, 2018 at 21450 CR 12 South, Foley, Al; breaking strength of lanyard: The employer failed to ensure employees used proper fall protection in that they were tying their lanyard directly to the D-ring on their harness which is not allowed and may result in the lanyard breaking during a fall.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) On or about August 23, 2018 at 21450 CR 12 South, Foley, Al; free fall distance:  The employer failed to ensure employees taking measurements and installing sheathing on the roof of a residential apartment building adjusted their rope grab or used a rope grab instead of tying their lanyard with a knot to the D-ring of their harness as they moved allowing additional slack to accumulate allowing a fall of more than 6 feet.

29 CFR 1926.1052(b)(2): Foot traffic was not prohibited on skeleton metal stairs where permanent treads and/or landings were to be installed at a later date and the stairs were not fitted with secured temporary treads and/or landings long enough to cover the entire tread or landing area:  a) On or about August 23, 2018 at 21450 CR 12 South, Foley, Al; skeleton metal stairs without permanent treads: The employer failed to ensure the skeleton metal stairs were temporary filled in or had the permanent treads were installed before employees were allowed to use them which exposed them to the hazards associated with trip/slip/fall while going up or down the stairs.
