29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: 885 Riverdale Avenue, West Springfield, MA: On or about August 23, 2018 employees were performing sheet metal ductwork installations from walking working surfaces that had floor holes which were not protected from employees tripping into or stepping through such holes by guard rails or standard covers. (a) Uncovered 60 inch deep in ground pool. (b) Mechanical chase ways on floor two, three and four which were located in the western stairwell not protected by guard rails or covers. (c) Elevator shaft on the first floor had no guard rails or other barriers to prevent employees from steeping into a 61 inch deep floor hole.
