29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Worksite: The employer did not ensure that the employee installing a sewer pipe in the trench measured as approximately 30 feet long, 12 feet wide at the top and 3 feet 7 inches wide at the bottom, and 11 feet 10 inches deep was protected from cave-ins by the use of appropriate cave-in protection system/device.
