29 CFR 1926.1053(b)(16): Portable ladders with structural defects were not immediately marked " Defective, Do not Use" and withdrawn from service: On or about August 20, 1918, at the worksite 1600 Samples Road, Pompano Beach Florida 33064, a ladder with a bent cross member that resulted from the ladder toppling over was not removed from service and remained stored in the equipment area and was positioned in a ready-for-use location. '
