29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval:  a) Plant - On or about 08/23/18 and at times prior thereto the yellow Hyster 50 LPG-powered forklift  (IMMEC #308) was operated with the front rack removed.  b) Plant - On or about 08/23/18 and at times prior thereto the yellow Cat Thompson LPG-powered forklift (IMMEC #317) was operated with the front rack removed.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Plant - On or about 08/23/18 and at times prior thereto the AccurPress hydraulic press brake IMMEC #717512 was not guarded.  b) Plant - On or about 08/23/18 and at times prior thereto the back of the AccurPress hydraulic press brake IMMEC #717512 was not guarded.  c) Plant - On or about 08/23/18 and at times prior thereto the Haco hydraulic press brake was not guarded.  d) Plant - On or about 08/23/18 and at times prior thereto the back of the Haco hydraulic press brake  was not guarded.  e) Plant - On or about 08/23/18 and at times prior thereto the  solid bar guard in the front of the Wysong hydraulic shear  (Mild 16 Stainless Max. Shear Capacity)  presented a 1.0 inch vertical gap, and a 2.0 inch horizontal distance to the clamp rods.   f) Plant - On or about 08/23/18 and at times prior thereto the back of the Wysong hydraulic shear (Mild 16 Stainless Max. Shear Capacity) was not guarded. 29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) Plant - On or about 08/23/18 and at times prior thereto the Jet pedestal grinder had a 1.25 in. gap between the left wheel and the work rest, and a 0.75 in gap between the right wheel and the guard. 29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  Plant - On or about  08/23/18 and at times prior thereto employees assigned to the structural shop operated the band saws and the grinders, and the employer did not monitor the level of noise in order to determine whether a Hearing Conservation Program was required. (Instantaneous readings ranged between 87.2 dB and 95.3 dB).
