29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition: On or about August 17, 2018, employees were installing wood blocking material between steel ribs and steel liner plates at Shaft #3 H-08 regulator shaft. Employees were using a hydraulic single acting cylinder ram (Model RC104) attached to a portable electrical Enerpac hydraulic pump unit (model PUJ1200B). Hydraulic oil was leaking from the base and pressure side of the cylinder ram because the bolt fastener was loose. The hydraulic unit on/off switch was broken and the identification label that included the capacity of the unit was missing.

29 CFR 1926.300(b)(4)(iii): Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided: On or about August 17, 2018, employees were exposed to a caught between hazard while using their hands to hold wood blocks in place at the Shaft #3 H-08 regulator shaft. The employees were using a portable electrical hydraulic unit with a single acting cylinder. The cylinder is used to press open the gap between the steel ribs and steel plates as the employees place the wood blocks between the steel ribs and steel plates to stiffen the structure. The employees were not using hand tools to hold the wood blocks in place.

29 CFR 1926.305(d)(1)(iv): Each jack shall be thoroughly inspected at times which depend upon the service conditions: On or about August 17, 2018, employees working at the Shaft #3 H-08 regulator shaft were using a portable electrical Enerpac hydraulic pump unit (model PUJ1200B) and cylinder ram (Model RC104) that was not thoroughly inspected upon use. The electrical start switch was broken, the cylinder ram was leaking hydraulic fluid and the manufacturers identification and capacity label was missing. The employer failed to inspect the unit [per the 1926.305(d)(1)(iv)(b) requirement] for jacks sent out of shop for special work, when sent out and when returned:
