29 CFR 1910.178(p)(1): Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition: a. On or about August 17, 2018, the green Combie lift had the directional controls on backwards and the lights nor horn worked and it was not taken out of service, exposing employees to crushing and struck-by hazards. b. On or about August 17, 2018, the yellow Combie lift horn did not work, exposing employees to crushing and struck-by hazards.
