29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a. On or about August 20, 2018, at the WestRock CP Holding Pond #2, the employer had not performed an analysis of the potential air concentrations to hydrogen sulfide gas emitted from discomposing organic sediment material bulldozed from the bottom of Holding Pond #2 to determine and implement feasible administrative and engineering controls to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d), exposing employees to high concentrations of hydrogen sulfide capable of causing nervous system disturbances and, moderate irritation to eye, skin, nose and throat.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a. On or about August 20, 2018, at the WestRock CP Jacksonville Holding Pond #2, employees using earth-moving heavy equipment to excavate organic material from the bottom of the pond had not been provided with effective information of the hazardous chemicals in their work area such as, but not limited to the toxicity, physical and health hazards and, hazardous levels of hydrogen sulfide gas emitted from the disturbed sediment material.
