29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which was incorporated by reference as specified in 29 CFR 1910.6: NEW BRITAIN FIRE DEPARTMENT HEADQUARTERS, 253 BEAVER STREET, NEW BRITAIN, CT: On or about August 24, 2018, an employee used a rope secured around the base of the neck of a compressed gas cylinder containing nitrogen to hoist the cylinder to the roof where he was working on the HVAC system. Once on the roof, the cylinder was placed in an upright position and was not secured to prevent it from being knocked over (CGA Pamphlet P-1-1965, Sections 3.2.4 and 3.3.8).

29 CFR 1910.102(a): The in-plant transfer, handling, storage, and utilization of acetylene in cylinders was not in accordance with Compressed Gas Association Pamphlet G-1-1966, which is incorporated by reference as specified in 29 CFR 1910.6: NEW BRITAIN FIRE DEPARTMENT HEADQUARTERS, 253 BEAVER STREET, NEW BRITAIN, CT: On or about and at times prior to August 24, 2018, an employee tied a rope around the neck of an acetylene cylinder to hoist it up to the roof of the building. There was no lifting attachment on the cylinder. Once on the roof, the cylinder was placed in an upright position and was not secured to prevent it from being knocked over (CGA Pamphlet G-1-2009, Sections 5.2 and 6.1).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: NEW BRITAIN FIRE DEPARTMENT HEADQUARTERS, 253 BEAVER STREET, NEW BRITAIN, CT: On or about August 24, 2018, the employer had not developed and implemented procedures to be followed when recovering R-22 (chlorodifluoromethane) refrigerant from HVAC systems to ensure that employees would not be exposed to hazardous chemicals such as phosgene gas due to incomplete deactivation of the system and removal of all refrigerant from the system.
