29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 24, 2018, during the installation of a storm sewer at a worksite located at 17001 Holland Road, Brookpark, Ohio, the employer did not ensure an employee working inside a trench with a depth of approximately 7.5 feet deep, was protected from cave-ins by an adequately designed protective system. The soil would have been classified as Type B .
