Employees used a forklift to elevate themselves while standing on a pallet connected to the fork tines. The employees used a fall harness and a shock absorbing lanyard to connect to the forklift mast, and this shock absorbing lanyard did not protect employees from striking the floor. This condition exposed employees to fall hazards of up to 16 feet.
