29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 22, 2018, at the northwest section of the jobsite an employee engaged in residential construction operations was exposed to a 22-foot fall hazard without means of fall protection.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid surface:  On or about August 22, 2018, at the northwest side of the jobsite an employee was exposed to a 8-foot fall hazard when the ladder used to gain access to the working level was not secured at it top to a rigid surface.
