(a)   Scaffold used to build stair case - On or about August 22, 2018 employees were exposed to a struck-by hazard from boards being passed overhead between employees. The employees were not wearing hard hats.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a)  Scaffold used to build stair case - On or about August 22, 2018 an employee was exposed to a flying object hazard while using a nail gun.  The employee was not wearing eye protection.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.    (a)   Scaffold used to build stair case - On or about August 22, 2018 employees were exposed to a fall hazard while working on a wooden scaffold with walkway less than 18 inches.  The scaffold walkway was constructed of single 6 inch and 8 inch wide boards and the scaffold was 12.7 feet above the ground.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a)  Scaffold used to build stair case - On or about August 22, 2018 employees were exposed to a fall hazard while working on a wooden scaffold without a suitable access ladder. The scaffold ladders steps were not parallel, level or uniformly spaced. Also a broken ladder was used for access to the scaffold.

29 CFR 1926.23: First aid services and provisions for medical care shall be made available by the employer for every employee covered by these regulations. Regulations prescribing specific requirements for first aid, medical attention, and emergency facilities are contained in Subpart D of this part.  (a)  On or about August 22, 2018 the employer did not provide first aid training or a first aid kit to his employees and the employer did not know the location of the nearest medical facility in case of emergency.
