29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) On or about August 24, 2018, workers were exposed to shock and burn hazards from an extension cord that was use in the construction area to charge a 50 amp battery starter, which had worn and ripped insulation exposing the energized conductors inside.
