29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 08/24/18, at a 2-story house located at 1572 Elmwood Ave., Buffalo, NY: An employee was standing on the aluminum pic of a ladder jack scaffold erected on the north side of the house while clearing debris from the roof tear-off from the pic. The employee was exposed to a fall of approx. 16 ft. to the ground because no fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 08/24/18, at a 2-story house located at 1572 Elmwood Ave., Buffalo, NY: Employees engaged in roofing tear-off work in various areas of the steep upper roof were exposed to a falls of approx. 10 to 20 ft. because no fall protection system was utilized. Pitch of the roof was approx. 5 in 12. NO ABATEMENT DOCUMENTATION REQUIRED All Access Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1175116, citation number 1, item number 1, and was affirmed as a final order on 10/12/2016, with respect to a workplace located at 4351 Arondale Drive, Williamsville, NY 14221.
