29 CFR 1926.451(d)(1): All suspension scaffold support devices, such as outrigger beams, cornice hooks, parapet clamps, and similar devices, did not rest on surfaces capable of supporting at least 4 times the load imposed on them by the scaffold operating at the rated load of the hoist (or at least 1.5 times the load imposed on them by the scaffold at the stall capacity of the hoist, whichever is greater): On or about 8/23/2018, Location: Top level and Exterior of The Prince Condominium; 1) Workers were performing balcony renovation activities from a swing stage scaffold with a motor rated hoist capacity of 1,000 lbs., only 950 lbs. of counterweights on each side, an arm with a reach of 37.5 inches, and a back span distance of 132 inches, thus exposing employees to a fall hazard.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity; On or about 8/23/2018, Location: Top level and Exterior of The Prince Condominium; 1) Four employees engaged in balcony renovation activities working from a swing stage suspended scaffold were exposed to fall hazards when the scaffold and the scaffold components were not inspected by a competent person before each work shift.

29 CFR 1926.502(d)(9): Lanyards and vertical lifelines did not have a minimum breaking strength of 5,000 pounds; On or about 8/23/2018, Location: Top level and Exterior of The Prince Condominium; 1) Workers performing balcony renovation activities from a swing stage scaffold were using lanyards as part of the fall arrest system and tied the lanyards with only a knot to a beam having sharp edges that had the potential of decreasing the strength of the ropes, thus exposing employees to a fall hazard.
