29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition:   (a) Family Dollar Store #350, Storage Room: On or about 08/27/2018, and at times prior, the employer exposed employees to slip and fall hazards, in that water from a HVAC leak from the mechanical closet was pooling on the work floor as employees were unloading and storing merchandise.

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) Family Dollar Store #350, Storage Room: On or about 08/27/2018, and at times prior, the employer exposed employees to fire hazards in that the emergency exit was locked from the inside using a barrel bolt sliding lock and an alarm locking dead bolt mechanism as the employees conducted inventory storage and stocking activities.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose.  (a) Family Dollar Store #350, Mechanical Closet containing the air conditioning unit located in the storage room: On or about August 27th, 2018, and at times prior, the employer exposed employees to electrical shock and/or electrical fire hazards in that junction boxes containing live wiring were left uncovered as the employees entered the location to retrieve a portable roller conveyor.  (b) Family Dollar Store #350, Storage room: On or about August 27th, 2018, and at times prior, the employer exposed employees to electrical shock and/or electrical fire hazards in that outlet boxes had broken or missing faceplates as employees performed inventory storage and retrieving in the location.
