29 CFR 1926.451(f)(7): Scaffolds were not moved or altered by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold moving or alteration and such activities were not performed only by experienced and trained employees selected for such work by the competent person: a) 1441 Chestnut Street, Philadelphia, PA 19102, 49th Floor - The employer, competent person, permitted an employee to position the extended platform of a powered mobile scaffold (scissor lift) in the path of an operating personnel hoist in a manner that did not avoid crushing hazards. This condition exposed the employee to struck-by and caught-in-between hazards. Noted on or about August 24, 2018.

29 CFR 1926.454(b): The employer did not have each employee involved in moving and operating a scaffold trained by a competent person to recognize any hazards associated with the work in question: a) 1441 Chestnut Street, Philadelphia, PA 19102, 49th Floor - The employer, competent person, permitted an employee to position the extended platform of a powered mobile scaffold (scissor lift) in the path of a personnel hoist without training the employee in the hazards associated with the process, thus exposing the employee to struck-by and caught-in-between hazards. This condition was noted on or about August 24, 2018.
