29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    a)  At the facility, furnace area: On or about August 25, 2018, energy control procedures for 1 Mega Watt arc furnaces did not outline steps to isolate electrical energy when employees performed service and maintenance tasks such as but not limited to threading new lengths of electrodes. This condition exposed employees to an electrocution hazard.

29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    At the facility, where employees were exposed to amputation hazards when operating the following equipment without guarding:    a)  Tail pipe and braided stainless steel hose trimming area: Alligator Shear, McIntyre, Model 407, unguarded at the point of operation.    b)  Foil Shredder Area: Band Saw, Tennewitz Type GN, Model GINE-RE, existing guarding is unable to guard the full range of blade exposure.    c)  Catalytic Converter Shearing area:  Shear blades, hydraulic, Techemet Model C5000, 2,500 psi, unguarded point of operation.    d)  Catalytic Converter Shearing area:  Openers, hydraulic, Techemet Model E8000, 1,500 psi, unguarded at the point of operation.
