29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.   a. On or about August 23, 2018, and at times thereto, employees performed electrical work on energized electrical equipment. The employer did not effectively train employees on electrical safe work practices to ensure safety from electrical shock and arc flash hazards.

29 CFR 1926.403(a): Approval. All electrical conductors and equipment shall be approved.   a. Located onsite at 3680 Easton Market in Columbus, Ohio: On or about August 27, 2018, a 120 volt romex cord connection was used as a flexible cord to temporary supply electric to the building outlets and lighting circuits from a portable generator. The unapproved connection and splice wrapped with electrical tape exposed employees to an electrical shock and fire hazard.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a. Located onsite at 3680 Easton Market in Columbus, Ohio: On or about August 23, 2018, employees were removing the load wires from an energized 480 volt meter socket. The employer did not provide personal protective equipment to protect employees from electrical shock and arc flash hazards.
