Instances A: On or about July 31, 2018, employees were exposed to falls up to approximately 11 feet above a work surface while operating a Skytrack Scissor Lift without the top and mid point gate being secured on the work platform of the lift. Instance B: On or about July 31, 2018, employees were exposed to falls up to approximately 16 feet above a work surface while operating a Skytrack Scissor Lift without the mid point gate being secured on the work platform of the lift.
