29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:   On or about August 27, 2018 during commercial roofing activities, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about August 27, 2018 employees were exposed to a fall hazards of approximately 25 feet.  The employees were using an aerial lift (Genie S 60) to gain access to and from the roof and were not properly attached with a fall harness and lanyard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about August 27, 2018 during commercial roofing installation activities, the employer did not ensure employees were protected from fall hazards of approximately 25 feet by ensuring a fall protection system/device was being used.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about August 27, 2018 during commercial roofing installation activities, the employer did not ensure employees were trained in the proper use and hazards associated with falls. Employees were observed not utilizing fall protection, wearing fall protection inappropriately, and not following manufacture recommendations for anchor points.
