29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about August 28, 2018 employees were exposed to a fall hazards of approximately 20 feet. The employees were using an aerial lift (JLG 450 AJ) to gain access to and from the roof and were not properly attached with a fall harness and lanyard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 28, 2018 at a worksite located at 7946 Darrow Road, Hudson, OH 44236, during roofing work on a steep slope roof, the employer did not ensure employees were protected from fall hazards approximately 10-20 feet by ensuring a fall protection system/device was being used. Country Meadows Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1244951, citation number 01, item number 02, and was affirmed as a final order on September 25, 2017, with respect to a workplace located at 3292 Blue Heron Trace, Montville, OH 44256.
