29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:    a) Nobo Construction, LLC, 5018 Mt. Pawnee, Frederick, CO  80504:  On and before August 21, 2018 the employer that each employee engaged in residential re-roof work at 19 feet was protected by personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  An employee fell 19 feet from a second tier roof and sustained fractures to the elbow and ankle and a partial collapsed lung.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  a) Nobo Construction, LLC, 5018 Mt. Pawnee, Frederick, CO  80504:  On and before August 21, 2018 the employer did not provide a training program for each employee who might be exposed to fall hazards.
