The quad box was not used in accordance with manufacturer instructions; on or about 8/27/18. c) Worksite, Dutch Express LLC: Quad box in dispatcher station was not mounted to a wall or beam. The quad box was not used in accordance with manufacturer instructions; on or about 9/26/18. Note: The employer is required to submit certification of abatement for this violation as normally required by 29 CFR 1903.19.
