On or about August 28, 2018, employees were exposed to a struck by hazard while installing vinyl siding materials on a residential home located at 2353 Norton Place, Avon, Ohio 44011. The employees were working from a pump jack scaffold above other employees working on the ground and the employer did not ensure the ground employees were wearing protective helmets.
