29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On August 29th 2018, at a job site located at 4140 Nolan Road, Independence, Missouri 64055, employees were exposed to an approximate 9 foot 4 and 3/8 inch fall hazard to the ground below in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection.   RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1104462, citation number 1, item number 1 and was affirmed as a final order on June 28, 2016, with respect to a workplace located at 1914 Main Street, Kansas City, Missouri 64108.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  On August 29 2018 at a job site located at 4140 Nolan Road, Independence, Missouri 64055, an employee was exposed to fall hazards of approximately 9 feet four and 3/8 inches to the ground below by accessing an elevated working surface area by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.     RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1104462, citation number 1, item number 3 and was affirmed as a final order on June 28, 2016, with respect to a workplace located at 1914 Main Street, Kansas City, Missouri 64108.
