29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the commercial construction site located at 4140 South Nolan Rd, Independence, MO 64055: Employees engaged in commercial roofing activities were exposed to fall hazards of approximately 9 feet from the awning to the ground below while accessing and exiting the roof. The portable ladder did not extend at least 3 feet above the upper landing and was not secured to the structure.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At the Commercial construction site located at 4140 South Nolan Road Independence, MO 64055: An employee was exposed to a fall hazard when no form of fall protection was provided to the employee working approximately 9 feet above the ground. An employee was removing old shingles and wood from the side of the roof near an awning, without the use of any form of fall protection. Complete Construction Service, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(14)), which was contained in OSHA inspection number 122336, citation 1, item number 1 and was affirmed as a final order on August 3, 2017, with respect to the to a workplace located at 2018 Main Street Kansas City, MO.
