(a) On the roof of the residential renovation project at 22 Russell Road Westfield, MA 01085, on 8/29/2018 employees were exposed to fall hazards in excess of 20 feet and the employer had not verified compliance with 1926.503(a) by preparing a written training certification record which included the name or other identity of the employee trained, the date of the training, and the signature of the person who conducted the training.
