29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Jobsite, 1308 Franklin Street, Johnstown, PA: On or about August 29, 2018, two employees were installing brackets on a commercial roof at heights of 30-60 feet above the ground and each one had their lanyard attached to the ring on the front of their harness and one employee did not have his leg straps attached.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: a) Jobsite, 1308 Franklin Street, Johnstown, PA: On or about August 29, 2018, two employees were working on a commercial roof at heights of 30-60 feet and were wearing fall protection, but each had the lanyard attached to one ring in the front of the harness and one employee did not have his leg straps attached. The employer said the employees have gone through fall protection training.
