29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a)    Norfolk Federal Building: On or about 08/27/2018, an employee cleaning skylights was not provided with fall protection, as a result was exposed to a 18' 3" fall hazard.  No abatement certification or documentation required.

29 CFR 1910.30(a)(1): The employer did not provide training to each employee who uses personal fall protection systems prior to being exposed to a fall hazard.  a)    Norfolk Federal Building: On or about 08/27/2018, an employee was cleaning exterior windows using a rope descent system that consisted of a boatswain chair and safety harness and was not provided with training prior to use.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a)    Norfolk Federal Building: On or about 08/27/2018, the employer failed to provide a written certification that a hazard assessment was conducted for the required use of a safety harness, safety glasses, ear plugs and steel toe shoes.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
