29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a)  On or about August 28, 2018,  an employee was exposed to fall hazard while connecting a horizontal support brace, while squatting on the Ferris wheel frame, and the employer did not ensure that appropriate fall protection was utilized.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about August 28, 2018, an employee working from a scaffold platform was exposed to a fall hazard and the employer did not ensure that the employee was trained to recognize the hazard and the steps necessary to minimize or eliminate the hazard.
