29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a)  On or about August 28, 2018,  an employee was exposed to fall hazard while connecting a horizontal support brace, while squatting on the Ferris wheel frame, and the employer did not ensure that appropriate fall protection was utilized.

On or about August 28, 2018, an employee working from a scaffold platform was exposed to a fall hazard and the employer did not ensure that the employee was trained to recognize the hazard and the steps necessary to minimize or eliminate the hazard.
