29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: a) Jeremy Grams, dba Grams Roofing, on or about August 29, 2018 at the address of 4125 E. Kirk Rd. Port Clinton, OH: The employer did not ensure employees were wearing appropriate eye protection, such as safety glasses while using a pneumatic nail gun to fasten roofing material and cleaning up debris on the ground. The employees using these tools in this condition were exposed to potential eye injuries from flying objects. Jeremy Grams dba Grams Roofing was previously cited for a violation of this occupational safety and health standard 1926.95(a) which was contained in OSHA inspection number 1237827, citation number 1, item number 1 and was affirmed as a final order on August 3, 2017 with respect to a workplace located at CR 140 Findlay, OH 45840.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Jeremy Grams, dba Grams Roofing, on or about August 29, 2018 at the address of 4125 E. Kirk Rd. Port Clinton, OH: The employer did not ensure employees were protected from falls while performing roofing activities from atop a commercial type structure. Employees were exposed to an approximate 15 ft. fall to the ground below. Jeremy Grams dba Grams Roofing was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.501(b)(13), which was contained in OSHA inspection number 1237827, citation number 1, item number 2, and was affirmed as a final order on August 3, 2017, with respect to a workplace located at CR140 Findlay, OH 45840.
