29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems.  15 WEST GRANBY ROAD, GRANBY, CT:  On or about August 28, 2018, employees were exposed to fall hazards while working in the attic of a house.  The employees were removing fiberglass insulation from between ceiling joists.  There was limited plywood for employees to stand or kneel on and no other fall protection provided.  An employee fell between ceiling joists and landed on the floor of the master bedroom below.  She succumbed to her injuries.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  15 WEST GRANBY ROAD, GRANBY, CT:  On or about August 28, 2018, the following equipment was not used in accordance with manufacturer instructions:  A)  A Leviton 698-W 15-Amp/125-Volt grounding triple outlet adaptor was used in concert with a flexible cord to energize a work lamp and a HEPA Air Scrubber.  B)  A Dri-Eaz F284 DefendAir HEPA 500 Air Scrubber equipped with a 3-prong plug was not plugged into a grounded receptacle.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4:  15 WEST GRANBY ROAD, GRANBY, CT:  On or about August 28, 2018, the employer used a Leviton 698-W 15-Amp/125-Volt grounding triple outlet adaptor to energize a yellow work lamp and a Dri-Eaz F284 DefendAir HEPA 500 Air Scrubber.  The Air Scrubber had a built-in outlet which was used to energize a Pro-Reel Fluorescent Work Light.  The total load on the adaptor was greater than 12 amps which is the maximum load specified in Table S-4.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  15 WEST GRANBY ROAD, GRANBY, CT:  On or about August 28, 2018, employees were using electrical equipment that did not have a continuous path to ground:    A) An orange extension cord used to energize equipment being used in the attic was missing a ground pin.  B) A Leviton 3-Way Power tap with only two wires was plugged into the extension cord noted above in order to energize work lamps and an air scrubber.  The air scrubber was equipped with a 3-prong plug.  C) A Dri-Eaz F284 DefendAir HEPA 500 Air Scrubber with a 3-prong plug was plugged into an ungrounded Leviton 698-W 15-Amp/125-Volt grounding triple outlet adaptor.
