29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On or about 08/30/2018, in the recycling area, the employer did not ensure that pallets stacked in an outside storage area were stacked in a safe manner. Stacks of pallets were leaning over to the extent that they had the potential to fall.

29 CFR 1904.5(a): The employer did not consider an injury or illness to be work-related when an event or exposure in the work environment either caused or contributed to the resulting condition or significantly aggravated a pre-existing injury or illness. Work-relatedness is presumed for injuries and illnesses resulting from events or exposures occurring in the work environment, unless an exception in �1904.5(b)(2) specifically applies: On or about 08/29/2018, the employer did not deem a fingertip amputation injury that happened in the Bakery (in 2017 or 2018) as work-related even though the event occurred in the inmate's work environment (work detail) and caused the resulting condition.
