29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval: On or about 08/29/2018, the employer did not ensure that forklift manufacturers provided written approval prior to allowing workers to use forklifts to pull multiple carts/trailers in series using the clevis hitch on the rear of the forklift.
