29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 30, 2018, during the installation of roofing material at a job site located at 6676 Willow Lake Drive North Royalton, OH 44133 the employer did not ensure employees working on a residential roof were protected from a fall of a height of approximately 8.9 feet on a 3:12 pitch roof and 18.0 feet on a 5:12 pitch roof by utilizing fall protection systems/devices. The employer did not train employees on how to recognize fall hazards and means of fall protection.
