This most recently occurred on August 29, 2018 on the jobsite located on 3909 S. 175th Ave, Omaha, NE 68130.  Five employees from the sub-contractor Oscar Miranda Roofing were observed working on the roof of a single-story house where there is a six in twelve steep sloped pitch at heights greater than six feet above the ground.
