29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The controlling contractor is failing to ensure that all employees of the sub-contractor Oscar Miranda Roofing are protected from fall hazards while working on a residential roof. This most recently occurred on August 29, 2018 on the jobsite located on 3909 S. 175th Ave, Omaha, NE 68130. Five employees from the sub-contractor Oscar Miranda Roofing were observed working on the roof of a single-story house where there is a six in twelve steep sloped pitch at heights greater than six feet above the ground.
