29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about August 30, 2018, employees at Cushioneer, Inc., were operating powered industrial trucks without successfully completing the training and evaluation as required by this paragraph. Employees were exposed to struck-by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: On or about August 30, 2018, the circular hand-fed ripsaws in the woodshop did not have guards on them which completely enclosed that portion of the saw above the table and above the material being cut. Employees were exposed to laceration and amputation hazards: a. The Craftsman saw did not have an affixed guard. b. The Delta saw did not have an affixed guard. Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).
