The employer is failing to ensure that the eyes and face of employees are protected from flying particles while working on a residential roof. This most recently occurred on August 29, 2018 on the jobsite located at 2227 S. 185th St, Omaha, NE 68130. Two employees were observed using powered tools to install roofing materials and not using any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on August 29, 2018 on the jobsite located at 2227 S. 185th St, Omaha, NE 68130. Four employees were re-roofing an existing single-story house. The roof has an eight in twelve steep sloped pitch and the ground to eve height is approximately ten feet. Nebraska Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1125822, citation number 1, item number 1, and was affirmed as a final order on March 11, 2016 with respect to a workplace located at George B. Lake Parkway, Omaha, NE 68120.
