29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and shall be constructed and loaded in accordance with that design. This violation was observed on or about August 30, 2018 at the 1100 N. Aspen Ave, Broken Arrow worksite: Employees were performing framing operations from a supported scaffold which exceeded the maximum width requirements when mounted on a GEHL Telescopic Handler exposing employees to the hazards of falls.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. This violation was observed on or about August 30, 2018 at the 1100 N. Aspen Ave, Broken Arrow worksite: A Green Country Drywall employee was observed working at approximately 15 ft. above the ground in an unsecured work platform on a GEHL Telescopic Handler exposing employees to fall and struck by hazards.
