Aspen, Broken Arrow, Oklahoma worksite: Employees were performing steel erection operations from a supported scaffold which exceeded the maximum width requirements when mounted on a Sky Trak rough terrain forklift exposing employees to the hazards of falls.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork: a) This violation was observed on or about August 30, 2018 at 2601 N. Aspen, Broken Arrow, Oklahoma worksite: M4 Construction & Welding, LLC employees were observed approximately 17 ft. above ground in an unsecured work platform on a Sky Trak 6036 rough terrain forklift, stenciled number 239332, exposing employees to the hazards of being thrown from the work platform. b) This violation was observed on or about August 30, 2018 at 2601 N. Aspen, Broken Arrow, Oklahoma worksite: M4 Construction & Welding, LLC employees were observed approximately 17 ft. above ground in an unsecured work platform on a Sky Trak 6036 rough terrain forklift, stenciled number 394259, exposing employees to the hazards of being thrown from the work platform.
