29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: Rig #4. On or about August 30, 2018, employees were working from the rig floor at a height of 6 feet without the use of a guardrail system or other forms of fall protection. This condition exposed the employees to a fall hazard.

29 CFR 1910.28(b)(11)(i): Each employee exposed to an unprotected side or edge of a stairway landing that was 4 feet (1.2 m) or more above a lower level was not protected by a guardrail or stair rail system: Rig #4. On or about August 30, 2018, stairways connected to the rig floor were not provided with handrails on each open side, exposing the employees to a fall hazard of more than 4 feet.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Rig #4: On or about August 30, 2018, the employer did not ensure the derrick or mast was provided with an auxiliary means of escape (Geronimo Line) and installed prior to allowing employees to work in the derrick. An employee was exposed to the hazard of fall from heights in the event of a fire or explosion.
