29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  At a residential construction site located at 20511 E Trinity Place, Blue Springs MO: Employees were exposed to electrical shock hazards. The employer failed to protect employee from electrical shock in that equipment was being used without a ground pin. This was demonstrated when a RIDGID R32104 brand circular (Serial # CS16032NC21945) saw was connected to an extension cord and did not have a ground pin to ensure the path to ground was permanent, continuous and effective.

Employees were working on attaching anchor points  to the roof for the jump-jack scaffold at an elevated height of approximately 25 feet without any means of fall protection. Employees were observed walking along the roof edges without any means of personal fall protection or fall protective systems.
