29 CFR 1926.34(c): Means of egress was not continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency. On or about May 4, 2018, the employer did not ensure that the means of egress from the building at Acme #6 out the exterior double doors was unobstructed. Employees were exposed to an approximate four to five foot fall into an excavated trench near the loading dock, when exiting from those doors.

29 CFR 1926.200(b)(1): A danger sign was not used where an immediate hazard exists. On or about May 4, 2018, the employer did not ensure that danger signs were posted on the inside of the double doors, warning employees that this was no longer an egress point and a hazard existed outside the doors. Employees were exposed to an approximate four to five foot fall into an excavated trench that was located immediately outside the double doors.
