29 CFR 1926.416(a)(1): An employee was permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: (a) Jobsite- Huffman Pipeline Project, School House Road, Waynesburg, PA: On or about September 2, 2018, an employee was electrically shocked when he contacted a 25kv overhead power line with a metal pig pole while taking a water sample on the top of a frac tank that was located underneath the overhead power lines.

29 CFR 1926.416(a)(3): Before work began the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring a person, tool, or machine into physical or electrical contact with the energized electric power circuit: (a) Jobsite- Huffman Pipeline Project, School House Road, Waynesburg, PA: On or about September 2, 2018, an employee contacted a 25kv overhead power line with a metal pig pole and was electrically shocked; before work began the employer did not identify that employees, tools, or machinery could come in contact with the overhead power lines at the jobsite.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Jobsite- Huffman Pipeline Project, School House Road, Waynesburg, PA: On or about September 2, 2018, employees taking samples on top of a frac tank were exposed to fall hazards of approximately 9'11" from the lowest point of the frac tank to the ground.
