29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Encore Riverwalk Apartments, Building 3: On or about August 14, 2018, and at times prior thereto, employees were exposed to slip, trip, and fall hazards while scrap lumber with protruding nails and other debris was not kept cleared from work areas and near a job-made wooden ladder that was used to access the roof.
