29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface. (a) STS Pit 6th floor- On or about September 26th, 2018 and at times prior thereto, employees were exposed to a falls of approximately 40' when working on the fish screen without a top rail on the north and south ends of the floor hatches. (b) STS Pit 4th floor- On or about September 26th, 2018 and at times prior thereto, employees were exposed to a fall of approximately 28' when working on the fish screen without a top rail on the north and south ends of the floor hatches.

29 CFR 1910.179(b)(6)(ii): Where passageways or walkways were provided, obstructions were so placed that the safety of personnel was jeopardized by movements the crane. (a) Powerhouse - On or about June 18th, 2018 and at times prior thereto, an employee was exposed to contact with the 385 ton EDERER bridge crane while working and transiting the crane rail while lifting and handling operations were taking place.
