29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:  On or about 05 September 2018, an employee engaged in a residential construction operation was not protected by guardrail systems, safety net systems, or a personal fall arrest systems. This exposed employees to a fall of approximately 25 feet.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level:  On or about 05 September 2018, at the subject job site, the employer failed to ensure that employees were protected from fall hazards in excess of 25 feet while engaged in the repair of masonary work on a scaffold built around a brick chimney.    Abatement documentation is required for this item in accordance with 29CFR1903.19(d).
