29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers was equipped with a stair rail system. Enamel Room. On or about August 27, 2018 through September 5, 2018, employees were exposed to fall hazards due to missing mid rails in the stair rail systems in the following locations: a) Powder Coat Booth "B", both sides. b) Access stairs between Enamel Room and Transfer Room. There is no abatement certification or documentation required for this item.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (c) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures: a) Enamel Room, Powder Coat Booths A, B & C. On or about August 27, 2018 through September 5, 2018 employees who performed servicing and maintenance tasks, including cleaning and adjustments to powder guns inside the booths, were exposed to struck by hazards from the overhead conveyor and pneumatic paint guns. Employees were not provided with adequate lockout/tagout procedures that contained a specific statement of the intended use of the procedure for cleaning and adjustments tasks, and did not contain a method for verifying deenergization of the pneumatic powder guns. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
